Evaluation of Aboriginal Health Case Management and Care Coordination models
We are working on a statewide evaluation of Aboriginal Health Case Management and Care Coordination models, looking at seven projects across the state, funded under the Koolin Balit strategy, for the Department of Health and Human Services. Evaluation of the Cubby House We are also undertaking an evaluation of the Cubby House at the Broadmeadows Children's Court for the Alannah and Madeline Foundation. |
